How to Log Into MyChart:

Using a smartphone

  • Download MyChart app (Android, iPhone)
  • Select “PPOC,” (Preferred Physicians of Children’s), as service area
  • Login with username and password

Using a computer

Via email

  • Open emailed link to access main MyChart login page
  • Select “Sign Up Now”
  • Input activation code, your date of birth, zip code, and email address
  • Choose username, password, and security question
